Referrals

Our goal is to provide same day or next day follow up for referrals from:

• Physician offices
• Emergency departments
• Hospital services
• Behavioral health specialists
• Peer Recovery Specialists
• Community service providers
• Other care partners for opioid use and other high risk substance use disorders.

Medicaid, Medicare and most major insurances accepted. If a person is not currently insured, they can still be seen and our team can help people obtain urgent coverage if qualified. Payment plans are available if needed. Please call for further information.

Please include the patient’s name, the patient’s phone number and reason for referral as well as your name, phone number, the referring institution and department if applicable
